We collect information directly from you, from healthcare providers involved in your care, and from health plans. This may include:

How We Use and Disclose PHI

Treatment
We may use and disclose your PHI, including your mobile phone number, to provide, coordinate, or manage your healthcare services. This includes communications related to scheduling, care coordination, and follow up instructions.

Payment
We may use and disclose PHI to bill and collect payment for services provided, including communications regarding billing questions or insurance matters.

Healthcare Operations
We may use PHI for routine operations such as quality assessment, accreditation, auditing, training, and compliance activities.

Communications by Text Message
If you provide a mobile phone number and consent to receive text messages, we may use SMS or RCS text messaging to send appointment reminders, billing notifications, care coordination messages, and administrative updates related to your services.We limit the content of text messages to non sensitive information whenever possible. SMS and RCS text messaging are not fully secure communication channels. You may opt out of receiving text messages at any time.

No Sale or Marketing Use of Mobile Numbers
Your mobile phone number will not be sold to third parties. It will not be shared with third parties for marketing or promotional purposes and will not be used for unrelated marketing communications without your express written consent.We may share your mobile phone number only with service providers directly involved in delivering communications on our behalf, such as messaging platforms and telecommunications providers, and only under confidentiality and security obligations.

Special Protections for Substance Use Disorder Records
If we create or maintain records protected under 42 CFR Part 2, those records are subject to additional confidentiality requirements. These may include written consent for certain disclosures, restrictions on use in legal proceedings, and additional opt-out rights for fundraising communications. These protections apply in addition to HIPAA requirements.
